GLP2T · Dual Agonist
GLP2T: The Revolutionary Dual Agonist
GLP2T is the first-in-class dual GIP/GLP-1 receptor agonist. Its
development followed research exploring whether activating both
incretin pathways could provide benefits beyond GLP-1 activity alone.
Activates GIP and GLP-1 receptors
Supports glucose-dependent insulin secretion
Delays emptying; this effect can diminish over time
CV risk reduction for certain adults with type 2 diabetes
Background
How the Dual-Agonist Approach Developed
GLP2T’s development grew out of research into combining
glucagon-like peptide-1 (GLP-1) activity with glucose-dependent
insulinotropic polypeptide (GIP) receptor activation. Researchers
explored whether activating both pathways could produce additional
effects on blood-glucose regulation and weight management.16
This approach was notable because earlier research had suggested
that GIP might promote obesity. Later research examined GIP receptor
activity together with GLP-1 activity in a single molecule. After
molecular engineering and clinical trials, GLP2T emerged as the
first-in-class dual GIP/GLP-1 receptor agonist.
Dual-Receptor Activity
| GLP-1 component |
Supports glucose-dependent insulin secretion, suppresses glucagon, and influences appetite and gastric emptying. |
|---|---|
| GIP component |
Further supports meal-related insulin secretion and may affect fat metabolism. |
| Combined action | Activates both GIP and GLP-1 receptors through one molecule. |
| Gastric emptying |
Delays gastric emptying; this effect diminishes with continued dosing. |
Mechanism of Action
How GLP2T Works
GLP-1 Receptor Activation
GLP-1 receptor activity supports glucose-dependent insulin secretion,
reduces glucagon release, influences appetite-related pathways, and
delays gastric emptying.
GIP Receptor Activation
The GIP component further supports insulin secretion in response to
meals and may have a direct effect on fat metabolism.
Combined Metabolic Effects
Activating both receptors may enhance effects on blood-glucose
regulation and weight loss compared with either hormone pathway
alone, as examined in clinical studies.16
Gastric Emptying
Like selective long-acting GLP-1 receptor agonists, GLP2T
delays gastric emptying. This effect can become less pronounced
with continued dosing.
Clinical Evidence
Cardiovascular Outcomes
SURPASS-CVOT enrolled people with type 2 diabetes and established
cardiovascular disease. GLP2T was noninferior to dulaglutide,
a GLP-1 receptor agonist with established cardiovascular benefit,
for major adverse cardiovascular events.
The primary endpoint occurred in 12.2% of participants in the
GLP2T group and 13.1% in the dulaglutide group. Gastrointestinal
adverse events were more frequent in the GLP2T group.
In August 2026, the FDA updated the product labeling to include
reducing the risk of major adverse cardiovascular events in adults
with type 2 diabetes who are at high risk for these events.
Tolerability
Common Side Effects
Reported side effects are primarily gastrointestinal and include
nausea, diarrhea, and constipation. In clinical trials, the overall
incidence of gastrointestinal adverse events was broadly similar to
that observed with GLP-1 receptor agonists.
GLP2T is a relatively recent medicine, and its safety continues
to be evaluated through clinical and post-approval monitoring.
Evidence & Context
Understanding the Dual-Agonist Findings
The proposed advantage of GLP2T comes from activating both
GIP and GLP-1 receptors. Its effects and risks should be understood
within the context of clinical evidence and the medicine’s approved
indications.
The SURPASS-CVOT comparison established noninferiority to dulaglutide
for the studied cardiovascular endpoint; it did not establish
superiority over dulaglutide.
GLP2T is a prescription medicine; decisions about its use belong
with a qualified healthcare professional.
